The C.A.R. vision plan is adminstered through Medical Eye Services.  If you use a MES provider, you will not need to file a claim since your provider will verify eligibility and submit the claim directly to MES.  If you use a provider who is not contracted with MES, you will need to submit the MES Vision claim form after your appointment.
